Patient Bill of Rights
All patients are entitled to:
1. Competent dental services without regard to age, race, color, sexual orientation, religion, marital status, gender, national origin or sponsor.
2. Considerate, respectful and confidential care.
3. An explanation of the services that are available at the center, the provisions for off-hour emergency care, the charges for services, eligibility for third-party reimbursement, and (when applicable) the availability of free or discounted care.
4. An itemized copy of your account statement, upon request.
5. Complete and current information concerning diagnosis, treatment and prognosis in terms the patient can easily understand.
6. Information necessary to give informed consent prior to the start of treatment, with an explanation of the possible risks associated with treatment as well as possible alternate modes of treatment.
7. Refuse treatment with a full explanation of the risks involved in electing not to be treated.
8. Notification of when experimental research is being conducted, and the option to choose whether or not to participate.
9. Voice grievances and recommend changes in policies and services without fear of reprisal.
10. Express complaint about the care and services provided and to have Dental First investigate. Dental First is responsible for responding to the patient, in writing, within 30 days as to the findings of the investigation. If the patient is not satisfied with this response, he/she has the right to complain to the Philippine Dental Association or any concerned agencies.
11. Privacy and confidentiality of all information and records pertaining to treatment.
12. Approve or refuse the release or disclosure of the contents of his/her medical record to any health care practitioner and/or health care facility.
13. Access to your medical record.
